LUKS cryptsetup: Cannot use device / already mapped

[PROBLEM]
Trying to mount a LUKS encrypted image file (over loop device). Worked flawlessly in the past (Ubuntu 12.04), but refuses to work in Ubuntu 16.04 :(
XFCE shows a crash when trying to mount it using the GUI.

Get the following error when trying to do it on the CLI (luksOpen):
Cannot use device /dev/loop1 which is in use (already mapped or mounted).
